Sec. 3. A petition seeking to restrain a person from contact must be entitled "In the Matter of a No Contact Order for ___________". The petition must allege the following: (1) That the respondent is likely to have direct or indirect contact with the child in the absence of an order under this chapter. (2) That the child has been adjudicated a delinquent child. (3) That the best interests of the child will be served if the person refrains from direct or indirect contact with the child.
